GME
credentials the following programs: ARNP, CRNA, CNM and PA.
GME does not provide housing. The list of required documents
can be found on the first page of the application. Advanced
Practitioners are responsible for finding their own preceptors
and obtaining signatures for approval. The GME Contracting
Coordinator will ensure preceptor applications/agreements
and academic affiliation agreements are in place. If these
are required the program coordinator will be contacted. All
documents are required within two weeks of the orientatio
and start date. An individual orientation session with the
Clerkship Coordinator cann be scheduled as soon as all documents
are received.
1. Advanced Practioners must complete the FH GME application
form found online and submit to the Clerkship Coordinator.
2.
Advanced Practioners must execute an affiliation agreement
between Florida Hospital and their training program prior
to the start of the rotation.
3.
Advanced Practioners must wear both their current program's
photo ID and FH ID badge at all times.
4.
Advanced Practioners must provide malpractice coverage for
the duration of their clerkship experience at Florida Hospital.
